Please take a moment to download, print, and fill out the new patient forms below prior to your appointment. If you are unable to print them at home, simply arrive 15 minutes early to your scheduled appointment to complete them in our office.

Your First Visit Checklist

  • Completed New Patient Forms (If filled out at home)
  • Valid Photo ID (Driver’s license or state ID)
  • Current Insurance Card(s) (Primary and secondary, if applicable)
  • List of Current Medications (Including dosages, vitamins, and supplements)
  • Any Relevant Medical Records or Imaging (Previous X-rays, MRI reports, etc.)
  • Your Current Footwear or Orthotics (If you are experiencing structural foot pain or gait issues)

Keeping Our Clinic Running Smoothly

At Momentum Foot & Ankle, our mission is to provide elite clinical care and aesthetic wellness to all our patients. To maintain a consistent schedule and accommodate those in need of urgent care, we ask that you review our key policies:

Cancellation & No-Show Policy: When a treatment time is missed, it prevents us from offering that specialized care to another patient. If you need to cancel or reschedule, please call our office at 843-954-3308 at least 24 hours in advance. Appointments missed or canceled with less than 24 hours’ notice will incur a $35.00 administrative fee.

Financial Responsibility: Co-pays, deductibles, coinsurance, and payments for non-covered services are due at the time of service. We will verify your insurance and submit claims on your behalf.

Podiatry & Clinic FAQs

What insurance plans do you accept?

We accept most major insurance plans, including Medicare, Blue Cross Blue Shield (BCBS), Aetna, Cigna, UnitedHealthcare (UHC), Tricare, and Humana. We also accept traditional Medicaid and select Medicaid managed care plans (such as Absolute Total Care, BlueChoice Healthy Blue, Humana Healthy Horizon, Molina, and Select Health – First Choice). Please call us to verify your specific Medicaid coverage.

Do I need a referral to see Dr. Fink?

In many cases, you do not need a referral to see a podiatrist. However, certain HMO insurance plans (including some Medicaid and BCBS plans) do require a referral from your Primary Care Physician before a specialist visit is covered. We recommend checking with your insurance provider prior to your appointment.

Do you offer telehealth appointments?

Yes! We understand that mobility issues or busy schedules can make it difficult to get into the clinic. We proudly offer telehealth services to meet our patients where they are, providing expert consultations from the comfort of your home.

What should I expect if I need surgery?

Dr. Fink always prioritizes conservative, non-surgical treatments first. If surgery is required, she specializes in advanced, minimally invasive techniques designed to reduce downtime and discomfort. During your consultation, she will walk you through the entire process, from the procedure itself to your customized recovery timeline.

Do you treat urgent foot and ankle injuries?

Absolutely. We treat acute injuries such as ankle sprains, sudden heel pain, and urgent wound care needs. If you have an urgent foot issue, please call our office directly to see if we can accommodate a same-day or next-day appointment. (Note: For life-threatening emergencies, always call 911 or visit the nearest emergency room).
